Patient Forms
When you book a new appointment at the Guelph Wellness House, you will be sent your online intake forms via email if you have provided one. Please fill out the appropriate form (adult or child) online prior to your appointment time. If you have trouble with the online form or do not have access to internet/email, please call us at 519-827-9605 or email at [email protected] to let us know and we will have them ready for you at the office 30 minutes prior to your first visit for you to fill out.
